Membership Application Form: April 2009 – March 2010

Please complete the form (click on the PDF Icon at bottom of page) following the instructions below :-

1. A cheque or postal order for £45 ( no cash ) payable to BA Clubs, include on the back
i . Staff members: STAFF NUMBER, WORK ADDRESS, GOLF SECTION
ii, Non Staff members: Bank Card Number with expiry date, GOLF SECTION

2. A passport size photograph of yourself. ( We create a Golf Section Card with Photo )

3. A self addressed envelope with stamp.

4. New members should supply a copy of their handicap certificate or 3 completed and signed scorecards. These can be supplied late but a BA Golf handicap cannot be issued until the scorecards are received. ( No Competition or Day Meeting will be allowed until the handicap certificate or 3 cards have been passed to the Handicap Secretary. )
